December 2011 -  East Sussex - DC and BC
Death of 2 brothers, aged 5 and 7, in a fire at their home in 2008.
Obsession with fire not known to agencies, impact of domestic violence on children’s behaviour, parental substance and alcohol misuse and impact on parenting capacity.

December 2011 -  Kirklees - Child aged 2 years 7 months
Death of a child aged 2 years 7 months at family home on 19 February 2010. Cause of death suggestive of smothering. Mother detained under Mental Health Act 1983. Maternal mental health (possibly schizophrenia).

November 2011 - Gloucester - SCR 0310
Life threatening injuries to Rachel, just under 2 years of age, in 2010. Mother sentenced to 2 years for 1 count of cruelty. Family newly arrived in UK. Language and integration problems. Use of neighbour as interpreter.

November 2011 -  Newcastle - Child B
Death of 16 year old from paracetamol ingestion.
Mother, father and sibling arrested for causing or allowing death of vulnerable person but no criminal proceedings.
Adolescent neglect, maternal mental health and disability, parenting capacity, young carer.

November 2011 - Blackburn with Darwen - Child H
6 month old infant died of bronchopneumonia in 2009 but 18 non-accidental injuries found.
Both parents convicted of child cruelty and sentenced to 5 years each.
Mother looked after child as teenager, teenage mother, honour based violence, social isolation.

November 2011 - Torbay - Child 24 (overview report)
2 year 8 month old girl died following cardiac arrest.
History of neglect, family violence and maternal substance misuse
Found a lack of understanding of neglect and interagency cooperation.

October 2011 -  Croydon - Child Z
A 2 year old boy admitted to hospital with a subdural haematoma and other non-accidental injuries in March 2010.
Asylum seekers, kinship care, assessment of parenting capacity.

October 2011 -  Rhonda Cynon Taff - Child K, Child N, Child R
Serious brain injuries sustained by 3 infants under 13 weeks. No connection between the three children.
Identification and assessment of neglect, dangers of injury from shaking babies, babies with complex health needs.

October 2011 - Durham - Child T
4 month year old baby boy who died of a traumatic brain injury in 2008. Second child removed at birth.
Lack of information sharing, premature birth, complex health issues.

October 2011 - Gwynedd and Anglesey - Child G and Child H
Family known to several agencies although no serious child protection concerns.
Family tension caused by contact disputes.

September 2011 - Leicester - Child 1 and Child 2
Deaths of 2 pre-school aged children with their mother in 2011. Father committed suicide.
Domestic violence, parental mental health, language and cultural issues. Increasing family pressure with debt and concerns about father’s immigration status.
Executive summary

September 2011 -  Vale of Glamorgan - Children C and D
Two children under ten years old sexually abused in 2008 by a 19 year old man with learning difficulties who was placed with their family by adult services.
The 19 year old was convicted of rape of a child aged under 13 and sexually assault on three occasions of another child aged under 13.
Learning difficulties - history of harmful sexual behaviour - lack of communication between service providers and the family.

August 2011 -  Lancashire - Child A
Death of child aged 16 found hanging in children’s home in 2006. History of domestic violence, substance and alcohol abuse, frequent house moves. Child with complex and multiple needs. Looked after since 11 years old, frequent placements due to challenging and inappropriate behaviour including violence and bullying.

August 2011 - Caerphilly - Child A (not available online)
15 year old boy died from hanging in his bedroom at home
Death by misadventure
Father died when child eight, mother's new partner's drug use and non-school attendance

August 2011 - Hull - Child F
13 month old child died from serious abdominal injuries in March 2010.
Mother's partner sentenced to minimum of 13 years for child's murder.
Domestic abuse, mother's new partner, family disengagement from universal services.

July 2011 - Caerphilly - Child K (not available online)
5 month old boy died in December 2009 from suspected non-accidental head injury.
Father contact with Police and Probation Service, domestic abuse from previous relationship. No concerns in current relationship.
Death not predictable or preventable, no lessons to be learned.

July 2011 - Worcestershire - Children BW and CW
Death of a child and the near death of their sibling after their father drove them into a river in 2010.
Parental mental health - Parental alcohol misuse - Parental separation
Need to follow interagency procedures and take into account children's needs.

June 2011 - Cumbria - Child C
Infant boy, few weeks old, sustained life-threatening injuries in September 2009.
Mother's boyfriend convicted of causing injuries, mother convicted of neglect.
Adolescent mother, domestic abuse.

June 2011 - Leeds - Child S
Boy, aged 11 years, died of asphyxia caused by ligature strangulation in 2009.
Coroner's verdict was misadventure.
Domestic abuse, financial problems, paternal alcohol abuse, uncooperative parents, child behaviour problems, child mental health.

[June 2011] - Wolverhampton - Child J
Child, aged 3 years, died from non-accidental injury.
Child was in the care of mother's friends who had histories of childhood abuse and one of whom was assessed as a risk to children.
Young mother, carers abused in childhood.

May 2011 - Cardiff - Child D
15 year old girl died of cardiac arrest in 2009 believed to be related to ingesting butane gas.
Looked after child, neglected teenager, emotional mental health, risk taking behaviour, substance abuse and relationship with older man. Mother at 15.

May 2011 - Monmouthshire - Child A
11 month old infant died from multiple stab wounds in November 2010.
Mother detained in hospital indefinitely.
No predictive factors that infant at risk of harm from mother. Includes brief indicative research review on filicide.

May 2011 - Sunderland - Child X
Male child arrested for committing a serious sexual assault.
Child mental health problems, child learning difficulties, young offender, bereavement at father's death, maternal alcohol and drug abuse, maternal mental health problems, asylum-seeking family, domestic violence.

April 2011 - Bristol - Child M (overview report)
Boy aged 2 years died in an incident at his grandmother's home.
Parents and maternal grandmother convicted of manslaughter and child cruelty.
Alcohol abuse, domestic violence, adolescent mother, maternal mental health.

11 year old boy committed suicide by hanging.

Abuse of four siblings by father between 2007-2009
Parents separating, contact issues, father's disguised compliance and an over-emphasis on the needs of the father, lack of appropriate assessments.

March 2011 -  Bournemouth and Poole - W.
3 adolescent boys convicted of manslaughter of a homeless adult male in 2009. All 3 families were hard to engage and there were missed opportunities for early intervention.
Young offenders, family support services, early intervention, information sharing, interagency co-operation.

March 2011 -  Bromley - Child D and Child B
2 brothers taken into care after one found to have injuries. Father found not guilty of assault.
Child neglect concerns, previous physical injuries, father with epilepsy, mother had been looked after and had learning difficulties, concealed pregnancies.

March 2011 - Merthyr Tydfil - Child 1
Infant boy, aged 38 days, sustained serious head injuries leaving him permanently disabled in November 2006.
Domestic violence.

March 2011 - Pembrokeshire - Child F
Child died by self inflicted asphyxiation
Bullying at school although not persistent problem
Death not predictable nor preventable.

February 2011 - Blackburn With Darwen - Child J
Infant, aged 5 weeks, sustained a head injury in June 2009.
Domestic violence.

February 2011 - Cardiff - Child C
Sudden infant death from co-sleeping in April 2009.
Risk factors associated with co-sleeping with young babies; domestic abuse; school attendance; disguised compliance and parental disengagement.

February 2011 - Cheshire East - Child B, Child C, Child D
Siblings aged 3, 2 and 6 months, emotionally abused and neglected by their adoptive parents between 2000 and 2009.
Parents convicted of child cruelty.
Parents lack of experience, professional parents, disguised compliance, highly resistant parents.

February 2011 - Northamptonshire - Child G/Tammy Nixon (overview report)
17 year old girl died from prescription drug overdose
Child in care since 6, vulnerability of care leaver, self-harm, previous overdose, lack of understanding of affects of early deprivation, over optimism.

Infant boy, aged approximately 6 months, found with old injuries when admitted to hospital for vomiting in April 2008.
Parents were charged with GBH and child cruelty.
Both parents abused as children, maternal alcohol abuse.

6 month infant died in family home, cause of death unascertainable.
Mother convicted of child neglect and sentenced to 16 months
Issues: maternal mental health and alcohol dependency; lack of focus on impact of these on parenting capacity; use of Common Assessment Framework.

Infant, nine and half months old, suffered convulsion due to ingesting adult prescription medication.
Mother and father not charged due to lack of evidence.
